  • Submuscular placement — Fully behind the pectoral muscle. Natural look, good coverage, slightly longer recovery due to muscle involvement.
  • Subglandular (over the muscle) — Faster recovery, no animation deformity, but higher capsular contracture rates. Less commonly recommended for primary augmentation.

2. Fat transfer breast augmentation

Fat is harvested by liposuction from donor areas (abdomen, flanks, thighs), processed, then re-injected into the breasts using ultra-fine cannulas. No implants, no foreign material — uses your own tissue.

  • Natural look and feel — no implant-related risks (capsular contracture, BIA-ALCL, rupture)
  • Simultaneous body contouring benefit in donor areas
  • Realistic size increase: 1–1.5 cup sizes per session
  • Requires adequate donor fat — not suitable for very lean patients (BMI below 20)
  • 20–45% of transferred fat is reabsorbed in the first months; results stabilise at 6–12 months

3. Hybrid augmentation (implants + fat transfer)

Combines a smaller implant with fat transfer to improve upper-pole softness and natural transition. Less commonly performed; confirm availability with your specific clinic.

What Realistic Results Look Like

Understanding what breast augmentation can and cannot deliver is the most important preparation step — more important than choosing the clinic.

How to read before & after galleries

  • Find patients with your starting anatomy — Look for “before” photos with a similar cup size, rib-cage width, skin laxity, and nipple position. A result on a different body type tells you very little about what your result will look like.
  • Check the timing of “after” photos — Many after photos are taken within the first weeks after surgery, before implants have fully settled. Look for galleries that specify the post-op date. Final shape takes 3–6 months to develop.
  • Same lighting, same angle, same distance — Front, both profiles, and 45° at minimum. A well-lit “after” next to a poorly-lit “before” is not an honest comparison.
  • Look for consistency across body types — A portfolio with varied body types showing consistent, proportionate results signals an adaptable surgeon. A gallery of identical-looking results on different patients is a warning sign.

What affects your final result

  • Starting anatomy — Skin elasticity, breast footprint, rib-cage shape, nipple-to-fold distance, and pectoral muscle development all influence the outcome more than implant choice alone.
  • “Natural look” depends on implant size relative to your frame — A very large high-profile implant on a small frame cannot look natural regardless of surgeon skill. Anatomy is the dominant constraint.
  • Implants do not lift the breast — Patients with ptosis (sagging) need a mastopexy with or without augmentation, not augmentation alone. Adding a lift is discussed at consultation based on the degree of droop.
  • Weight changes affect results long-term — Significant weight gain or loss after surgery will change the appearance of the augmented breast. Pregnancy and breastfeeding also affect results.

The “drop and fluff” timeline

Immediately after surgery, implants sit high and the upper pole looks full or boxy. This is normal — the pectoral muscle is tight and swelling is present. As healing progresses:

  • Weeks 1–2 — Implants sit high. Swelling and tightness. The result at this stage is not representative of the final outcome.
  • Weeks 3–6 — Implants begin to descend. One side often settles ahead of the other — this self-corrects. Lower pole begins to fill out.
  • Months 2–3 — Noticeable softening. “Fluffing” of the lower pole becomes visible.
  • Months 3–6 — Majority of settling complete. Approximately 90% of the final shape is visible by month 6.
  • Months 6–12 — Subtle final refinement. Scar maturation. Final result established.

Psychological readiness

A good clinic will ask about your motivations at consultation. Research consistently shows that patients who proceed for internal reasons — personal confidence, not external pressure — report significantly higher long-term satisfaction. Patients with untreated body dysmorphic disorder (BDD) do not benefit from surgery and should be referred for psychological support first. Arrival in Belgrade

Video consultation at 2–3 weeks and 6–8 weeks. Any concerns — asymmetry, unexpected swelling, wound questions — are addressed remotely. Most Belgrade clinics offer at least two online follow-ups included in the package.

Am I a Good Candidate?

Breast augmentation delivers the best outcomes for patients who:

  • Are in good general health with no active infections or uncontrolled systemic conditions
  • Have realistic expectations — improvement in size, shape, or symmetry, not transformation into someone else’s body
  • Are not pregnant or breastfeeding, and are not planning pregnancy in the near future

Think carefully before proceeding if: you have resolved or unresolved findings on recent mammogram or ultrasound, uncontrolled diabetes, BMI above 40, active nicotine use within 6 weeks, or pregnancy planned in the near future. Implants are not lifetime devices. Modern implants typically last 10–20 years; there is no mandatory replacement schedule, but patients should be aware they may need further surgery at some point. This is a long-term commitment that deserves careful consideration.

💡 Modern silicone implants are not affected by cabin pressure. The concern about flying after breast augmentation relates to DVT risk and wound healing — not implant integrity. Wear graduated compression stockings, stay hydrated, and walk the aisle every 60–90 minutes on flights over 2 hours.

Recovery — What to Expect

Day 1

Tight “band” sensation across the chest — especially with dual-plane placement where the pectoral muscle is involved. Anaesthesia grogginess, possible nausea. IV and oral pain control. Gentle walking from the same evening to reduce DVT risk.

Days 2–5

Round vs Anatomical — How to Choose

A landmark randomised controlled trial found no consistent aesthetic superiority of anatomical over round implants in routine augmentation. Both achieve natural results when matched appropriately to the patient’s anatomy.

  • Round implants — Symmetrical. If they rotate, no visible difference. More consistent projection in the upper pole. Lower cost within the same brand. Suitable for the majority of primary augmentation patients.
  • Anatomical implants — Teardrop shape with more volume in the lower pole. Can create a more graduated upper-pole appearance on patients with minimal existing breast tissue. Rotation is theoretically visible — though modern surface designs minimise this risk significantly.

Implant Surfaces & BIA-ALCL

BIA-ALCL (Breast Implant-Associated Anaplastic Large Cell Lymphoma) is a rare lymphoma associated primarily with macro-textured implants. As of June 2024, the FDA had recorded 1,380 cases worldwide across all implant types — 86% linked to Allergan BIOCELL macro-textured implants, which were globally recalled in July 2019.

The 2024 AAPS consensus (Plastic and Reconstructive Surgery, September 2024) recommends discontinuing macro-textured implants (surface roughness above 50 μm). Belgrade clinics have aligned with this: smooth and micro-textured implants only are placed. Polytech Microthane has recorded zero BIA-ALCL cases in UK patients per MHRA data. Motiva SmoothSilk has recorded zero confirmed BIA-ALCL cases across 3 million+ implants placed since 2010.

Key risks to understand:

  • Capsular contracture — The body forms scar tissue around the implant that can tighten progressively.   • Reoperation rate — Approximately 20–36% of primary augmentation patients have at least one reoperation by 10 years. The most common reasons are patient-requested size change, capsular contracture treatment, and implant rupture.
  • Breast Implant Illness (BII) — A collection of systemic symptoms reported by some patients with implants. Not a formally recognised diagnosis. FDA mandates a boxed warning on all breast implants acknowledging these reports. 87.5% of patients who request explantation report symptom improvement.
  • Infection, haematoma, seroma — Less common complications. Antibiotic prophylaxis and sterile technique minimise infection risk. Haematoma (blood pooling) and seroma (fluid accumulation) may require drainage.

    FAQ — Breast Augmentation in Serbia

    How long do breast implants last?
    Modern silicone implants typically last 10–20 years. There is no mandatory 10-year replacement schedule — replace when clinically needed (rupture confirmed, capsular contracture causing discomfort, or patient-requested size change). The Mentor 10-year Core Study showed approximately 80% of primary augmentation patients still had their original implants at 10 years. Plan for the possibility of at least one revision surgery during your lifetime.
    Which implant brand will I get?
    This depends on the clinic and is confirmed at consultation. Most Belgrade clinics include Mentor MemoryGel or Polytech MESMO as standard in their quoted prices. Motiva is available at select clinics at a premium. Always ask specifically which brand is included in the quote — this is the biggest variable in pricing and quality.     What is BIA-ALCL and should I be concerned?
    BIA-ALCL is a rare lymphoma associated with macro-textured breast implants — specifically Allergan BIOCELL, which was globally recalled in July 2019. As of June 2024, 86% of the 1,380 confirmed cases worldwide were linked to Allergan BIOCELL. Belgrade clinics place smooth and micro-textured implants only (in line with the 2024 AAPS consensus). Motiva SmoothSilk has recorded zero confirmed BIA-ALCL cases across 3 million+ implants. Your risk with the implants currently used in Belgrade is extremely low.
    Does my manufacturer warranty work back home?
    Yes. CE-marked implant manufacturer warranties apply globally — they cover the implant, not the surgery location. Mentor’s LifeTime Comfort warranty, Polytech’s Implants of Excellence lifetime warranty, and Motiva’s warranty programme all apply to implants placed by certified surgeons in Belgrade. Keep your implant registration card and operative notes for any future warranty or replacement claims.
    Can I breastfeed after breast augmentation?
    Most women can breastfeed after breast augmentation. The inframammary incision (standard in Belgrade) does not affect milk ducts or nipple sensation. Periareolar incisions carry a higher risk of affecting breastfeeding — this approach is not standard in Belgrade. Discuss breastfeeding plans at consultation, particularly if you are planning pregnancy in the near future.
    How soon can I fly home after breast augmentation?
    Short-haul flights (under 4 hours) are typically cleared at the day 5–6 wound check with written fit-to-fly clearance from your surgeon. Long-haul flights (over 4 hours) are recommended from day 10–14. Modern silicone implants are not affected by cabin pressure. The concern is DVT risk during prolonged sitting — wear graduated compression stockings, stay hydrated, and walk the aisle every 60–90 minutes. Do not book your return flight before receiving fit-to-fly clearance.
    Can I combine breast augmentation with a breast lift?
    Yes — augmentation-mastopexy (implants combined with a lift) is performed at Belgrade clinics. It is a more complex procedure and typically adds €1,000–€2,000 to the total. Plan for 8–10 nights in Belgrade rather than 6–7. Your surgeon will advise at consultation whether a lift is needed based on the degree of ptosis (sagging). Implants alone do not lift the breast — if you have sagging, augmentation without a lift will not correct it.
    What about Breast Implant Illness?
    Breast Implant Illness (BII) refers to a range of systemic symptoms — fatigue, brain fog, joint pain, hair loss — reported by some women with breast implants. It is not a formally recognised medical diagnosis and the causal mechanism is not established. The FDA mandates a boxed warning on all breast implants acknowledging these reports. 87.5% of patients who choose explantation report symptom improvement. Discuss BII concerns openly with your surgeon before proceeding.
    How do I verify my surgeon is qualified?
    Ask for the surgeon’s registration number with the Serbian Medical Chamber (Lekarska komora Srbije) and their SRBSAPS membership. SRBSAPS is affiliated with ISAPS (International Society of Aesthetic Plastic Surgery) and EASAPS. You can search the ISAPS member directory at isaps.org. Ask about annual breast augmentation case volume, board examination details, and request to see before-and-after photos of their own cases. A qualified surgeon will provide all of this without hesitation.
    Is B-Lite available in Belgrade?
    Yes, at select partner clinics. B-Lite is a Polytech lightweight implant approximately 30% lighter than standard silicone. It requires custom manufacturing with a twelve-week lead time — plan your surgery date accordingly. The 5-year study showed 97.1% of patients were reoperation-free.
