2. Lower blepharoplasty
Addresses under-eye bags caused by fat prolapse and, in some patients, excess lower lid skin. The technical approach is more variable than upper lids — and the decision significantly affects both the result and the risk profile.
- Transconjunctival approach: incision inside the lower lid, no external scar — best for fat prolapse with minimal skin excess and good lid tone
- Transcutaneous (external) approach: incision below the lash line — indicated when meaningful skin laxity is present; higher risk of lid retraction than transconjunctival
- Fat repositioning vs removal: modern practice increasingly favours repositioning fat to fill the tear-trough hollow rather than simply removing it — removal alone can create a hollowed appearance over time
3. Combined upper and lower blepharoplasty
Both upper and lower lids addressed in a single procedure.
- Saves cost and avoids two separate recovery periods
- Usually performed under light sedation or general anaesthesia (not local alone)

4. Brow position — why some patients need a brow lift instead
A low brow position can mimic or exaggerate the appearance of upper lid excess. Removing more upper lid skin in this scenario gives a poor result.

- Under-eye fat bags creating a persistently tired or aged appearance
- Good general health; no active eye infections or untreated eye conditions
- Realistic expectations — blepharoplasty refreshes the eye area, it does not change your fundamental facial structure or erase pigmentation-related dark circles
- Not currently pregnant; stable weight
Extra caution if: you have dry eye syndrome (blepharoplasty can temporarily worsen symptoms — discuss with your surgeon and have dry eye assessed before surgery); thyroid eye disease; previous eyelid surgery; or significant lower lid laxity (which may require canthopexy to prevent ectropion).
Blepharoplasty vs non-surgical alternatives
Botox can lift the brow slightly and reduce crow’s feet. Filler can address under-eye hollowing but not fat prolapse. Neither reliably addresses true skin excess or herniated fat — those require surgery. If you have significant upper lid hooding or prominent eye bags, non-surgical options are unlikely to produce the improvement you are looking for.

Safety & Risks
Blepharoplasty has a good safety profile in experienced hands. Understanding the risks before surgery is part of informed consent.
Blepharoplasty is generally considered one of the lower-risk facial plastic surgery procedures. Serious complications are uncommon — but they exist and must be understood before proceeding.
Dry eye and irritation are among the most common post-operative complaints, particularly in the first weeks. Temporary difficulty fully closing the eyelids (lagophthalmos) can occur as swelling resolves and typically improves within weeks. Patients with pre-existing dry eye must be assessed before surgery and should discuss this explicitly with their surgeon.
Lower lid malposition (ectropion) — outward turning of the lower lid — is the most significant aesthetic complication of lower blepharoplasty. It is more common with transcutaneous approaches and can require revision surgery. Risk is significantly reduced by appropriate patient selection, conservative skin removal, and canthopexy where lower lid laxity is present.
Haematoma (blood collection) is uncommon but possible, more so in patients taking blood-thinning medications. Stop aspirin, ibuprofen, and relevant supplements at least two weeks before surgery and disclose all medications.
Scarring in upper lid blepharoplasty sits within the natural lid crease and typically becomes near-invisible. Lower transcutaneous scars lie just below the lash line and fare similarly well. Both mature over 12 months.
Vision changes are rare. A very rare but serious complication is retrobulbar haematoma — bleeding behind the eye — which is a surgical emergency. This is extremely uncommon in routine blepharoplasty and is best managed by operating in a facility with the capacity to respond promptly.
How to verify credentials and facility standards in Serbia
Surgeon licensing in Serbia is obligatory and verifiable. The Lekarska komora Srbije (Serbian Medical Chamber) publishes a registry of doctors with valid active licences — any surgeon you consider should be findable here before you book. Facilities must meet Ministry of Health conditions (staff, equipment, premises) under Serbian law. Accreditation through Agencija za akreditaciju zdravstvenih ustanova Srbije is an additional quality signal when held — ask the clinic directly.
Ask any Belgrade clinic whether structured pre-operative and intraoperative safety checklists are in use. Do I need upper, lower, or both?
Upper blepharoplasty addresses skin hooding and heaviness over the upper lids. Lower blepharoplasty addresses under-eye fat bags. Many patients only need one — having both done when only one is needed can create an over-operated appearance. Will blepharoplasty remove my dark circles?
